I have reviewed the club's email dated October 31, 2018 and accompanying materials regarding the changes in ownership as of December 1, 2018 and my club membership options going forward. I understand that my current membership provides access to all three club locations in Lodi, but as of December 1, 2018 I will only have access to the single club location that I select should I continue my membership.
I understand that this response form will be date and time stamped when submitted and that my response must be received by November 15, 2018 to be processed by the end of November. I also understand that if I would like to make changes in my membership (e.g., address change, add or remove family members from my membership, etc.), I will need to contact the club directly to make those changes.
